A Cost-benefit Analysis of Using Polyethylene Glycol Hydrogel Sealant versus Fibrin Glue as a Dural Sealant for Posterior Fossa Surgery in the United States

چکیده
Background: Cerebrospinal fluid (CSF) leaks are a well-known complication of posterior fossa neurosurgery. The use of dural repair adjuncts has been associated with fewer CSF leaks and subsequently, lower medical costs. Objectives: To determine if the use of polyethylene glycol hydrogel sealant (DuraSeal dural sealant system [DuraSeal]), resulted in cost savings in the United States compared to use of fibrin glue in posterior fossa neurosurgery based on a published study of these two products. Methods: Primary analysis was based on a published cohort study (100 patients per cohort). Adjustments were made to account for the longer follow-up time of the fibrin glue cohort to better capture the incidence of CSF leaks, pseudomeningocele, wound infection, and meningitis using additional data from the literature. Resource utilization was calculated from literature studies and confirmed with consultations from two practicing neurosurgeons. The effective time horizon was 19 months. Undiscounted Medicare payments were used to calculate unit costs from a payer’s perspective based on hospital stay, cost of sealant application, and cost of complications. One-way sensitivity analysis was used to examine changes in costs as a result of model input changes. Results: In the base case, the cost of the hospital stay for the original surgery, which excludes the cost of complications, dominated costs for both cohorts. On a per patient basis, the use of fibrin glue cost $1666 more than the use of DuraSeal. Sensitivity analysis showed that using lumbar drainage instead of operative repair for a CSF leak reduced cost savings to $680. Holding the incidence of CSF leaks constant for the fibrin glue cohort while increasing the incidence for the DuraSeal cohort by 50% or 300% resulted in cost savings of $1438 and $755, respectively. Conclusions: The results of this study demonstrate a positive, consistent cost-benefit of DuraSeal compared to fibrin glue based on a cohort study of real world patients who underwent posterior fossa neurosurgery. 1Strategic Solutions, Inc., Cody, WY, USA *Corresponding author [email protected]

Delayed Cerebrospinal Fluid Leak after Watertight Dural Closure with a Polyethylene Glycol Hydrogel Dural Sealant in Posterior Fossa Surgery: Case Report
A polyethylene glycol (PEG) hydrogel sealant recently has been approved as an adjunct to sutured dural closure in Japan. We treated consecutive six patients with PEG hydrogel sealant in posterior fossa operation. Three of six cases suffered delayed cerebrospinal fluid (CSF) leak after watertight dural closure with the PEG hydrogel sealant, although there was no leak case which was treated with ...
متن کاملTwo alternative dural sealing techniques in posterior fossa surgery: (Polylactide-co-glycolide) self-adhesive resorbable membrane versus polyethylene glycol hydrogel
BACKGROUND Post-operative cerebrospinal fluid (CSF) leak in posterior fossa surgery remains a significant source of morbidity. TissuePatchDural (TPD), a novel impermeable adhesive membrane, was used to reinforce dural closure. A comparison with one of the most commonly used dural sealing devices, DuraSeal, has been made. METHODS A retrospective, single-center study was conducted on 161 patien...
متن کاملPreliminary investigation of a polyethylene glycol hydrogel "nerve glue"
BACKGROUND Polyethylene glycol (PEG) hydrogel is a biocompatible semi-adherent gel like substance that can potentially augment nerve repair much like a fibrin sealant. Potential advantages of this substance include fast preparation and set up time, as well as adhesion inhibiting properties. The purpose of this study was to perform an initial evaluation of PEG hydrogel in this application. MET...

OBJECTIVE Incisional cerebrospinal fluid (CSF) leakage after cranial surgery is a significant cause of morbidity due to poor wound healing and infection, meningitis, and pseudomeningocele formation. Many common dural closure techniques, such as sutures, autologous grafts, gelatin or collagen sponges, and fibrin glues, are used to achieve watertight closure, although none are US Food and Drug Ad...

INTRODUCTION Dural defects and cerebrospinal fluid (CSF) leaks are a common problem following posterior fossa surgery. The management includes either nonoperative management (e.g. external lumbar drainage) or surgical re-exploration. OBJECTIVES We present our surgical, clinical and histopathological experience of dural closure in the posterior fossa. In order to avoid CSF leaks we developed a...
